New Project Fact Sheets are works in progress just as new projects are, and will be modified periodically to accurately reflect changes in the details and progress of any particular development.   In an effort to provide progress reports as our projects unfold, we need to note that all information found here should be considered preliminary in nature and subject to change unless otherwise specified.

Project Title: Centennial Residential Treatment Home

Anticipated Opening Date: March 2010

Type of Facility/Specifics:

Centennial is a Residential Treatment Home (RTH).  RTHs are state licensed, non-locked, community-based homes with a service capacity of 5 or less adults with mental illness.  Each RTH is specifically designed (or modified), constructed and staffed based on the population served.

Resident Population To Be Served:

Two adults who  have a mental illness and who are ready and appropriate for living in an integrated community setting with the appropriate supports and treatment services.

General Development Updates:

ColumbiaCare has made some slight modifications to the existing home in order to meet state licensing and ADA requirements.

Screening and Placement for Residents:

Prior to admission, potential residents will have been screened for appropriateness by ColumbiaCare/Centennial staff, Multnomah County and the State of Oregon Addiction and Mental Health Division.  All parties must agree that the proposed resident is likely to feel safe and hopeful about the placement, fits in with the other resident(s) of the home, and is likely to benefit from the therapeutic environment and services offered with in the home.

Staffing and Oversight:

Centennial RTH will be staffed with awake staff 24 hours a day, 7 days a week.

ColumbiaCare understands that members of the community may have questions about Centennial RTH and the residents living there.  We are happy to answer questions about our company and the home, but are required by law to safeguard the rights and privacy of our residents (under the Health Insurance Portability and Accountability Act (HIPAA), and the Americans with Disabilities Act (ADA)/Federal Fair Housing).
