4-Bed Community-Based Residential Treatment Home

Individual Resident Population To Be Served:

4 Individuals who have a chronic mental illness, and who are ready and able to live in licensed, community-based residential program, but who require treatment and supports for psychiatric, minor behavioral and assistance with activities of daily living.

Staffing and Services:

To be determined.

Progress Report:

January 3, 2012:  ColumbiaCare has received notice of award for funding the project and will begin identifying appropriate potential properties for development.

5-Bed Community-Based Residential Treatment Home

Individual Resident Population To Be Served:

5 individuals who have a chronic mental illness, are ready and able to live in licensed residential program in the community, but who require a moderate level of medicare care for physical and mental health needs.
