Project Title: Bay Apartments II

Location: Coos Bay, Oregon

Anticipated Completion Date: September 1st, 2010

Type of Facility/Specifics:

Bay Apartments II is a 6-unit supportive housing program that will provide affordable and comfortable homes for individuals living with a mental disability, and who can live successfully in the community with minimal supports and community services. ColumbiaCare will be providing on-site management and will be partnering with Coos County Mental Health for the provision of support services, as needed.

Resident Population To Be Served:

Citizens of Coos County who are receiving mental health services

Staffing and Services:

ColumbiaCare will have an on-site Resident Manager who manages the property.  The Resident Manager will work together with a Recreational Case Manager and Coos County Case Manager to create an individual service plan for each tenant to promote successful, semi-independent living.

Estimated Development/Communication Timeline:

The Bay Apartments II Open House will be held on August 17th from 4pm-6pm.
