Type of Facility/Specifics:
ColumbiaCare is providing six individual units of HUD-approved, supportive housing for veterans and their families. We are coupling low-income housing with family counseling and readjustment support services to help during the transition from active service to civilian life–whether this be a short or long-term process.
Individual Resident Population To Be Served:
Veterans and families in need of mental health and readjustment support services.
Staffing and Services:
ColumbiaCare’s is working in partnership with the VA Southern Oregon Rehabilitation Center to coordinate the referral and screening process. While the property management function is carried out by our Administrative Office, services are delivered through a combination of both our own Veterans Services Program Manager and through contract with a licensed therapist.
